I am in a similar situation with my manager... she is good at managing projects, but not so much at managing people. My review (and raise!) is now 1 month overdue and I was getting more and more frustrated that it kept getting pushed off. After reading your post, I sent out yet another email requesting that my review get scheduled asap, but this time copied her boss, which worked--my review is now scheduled for this week. I'm not sure if that was the right tactic--she was probably irritated that I got her boss involved, but at least my review is scheduled!
